We require that you fill out the application packet in our office, so please set aside 20-30 minutes.

Some of the documents we require to see are:

- Driver’s License or California ID

- Social Security Card

- 2 Letters of Recommendation

- TB/PPD Skin Test (we will send you if you do not have one)

- Physical Exam (we will send you if you do not have one)

- CPR card

- Auto Insurance

- Auto Registration
