Medication Monitoring

Studies show that between 40%-75% of older adults do not take their medications properly.  At this age we are more sensitive to drugs because of our slower metabolisms and less effective organ function.  Seniors also have more trouble remembering what medications they have or have not taken.

A substantial risk for older adults is an adverse drug interaction of two or more drugs.

Below is a sample list of Medication Treatment Risk Factors:

- Taking 5 or more medications.

- Inability to manage medication-related devices.

- Taking 12 or more doses per day.

- Difficulty caused by language barriers, dexterity problems, poor eyesight or cognitive impairment.

- Significant changes in medication treatment regimen in last 3 months.

- Prescriptions from more than one physician.

- Medications requiring therapeutic monitoring.

- Recent discharge from a facility.

- Sub-optimal response to drug treatments.

- Suspected non-compliance.

Astera Care offers a comprehensive monitoring system designed to mitigate the dangers posed by drug-related errors.

Our Medication Management program ensures drugs are taken properly, on a set schedule and in coordination with the prescribing physicians’ orders.
