- Facility Monitoring:Astera Care is available to monitor a loved one in an institutional setting and to act as an advocate to their general care program.-Physician Interface: Our Certified Care Managers can work with the clients’ physicians to ensure that relevant information is flowing to the appropriate parties. Some clients may have difficulties remembering recommendations given by a physician, and a Care Manager’s participation in this information exchange can be invaluable. It ensures that a physician’s directives are carried out and that any pertinent information regarding the client’s condition is conveyed to the appropriate health care professionals.
