Custom Care
Astera Care has since 2003 offered our clients our traditional model of providing customized home care. Today we also offer our memberships with a set amount of monthly caregiver hours and The Concierge Companion with its safety and communications features. Still, the compassionate in-home care is why our clients trust us with their care needs.
What is Custom Care at home?
Custom Care in the home means something different to every client. At Astera Care we believe in customizing each client’s Care Plan by taking into consideration the concerns of the client, the client’s family and other associated parties. This allows us to design a Care Plan that promotes safety, comfort and peace of mind for everyone involved. Care may include in-home caregivers, or any combination of other services.
How is Care Customized?
- A Care Manager first schedules a time to meet with the client and the client’s family, and interviews all parties involved.
- The Care Manager performs an In-home Assessment, which involves evaluating the client’s physical, mental and psychosocial status, along with the client’s complete medical history.
- Upon completion of the In-home Assessment, the Care Manager writes a detailed Care Plan, taking into consideration the wishes of the client and the client’s family. The Care Plan determines the best course of action for each individual client, addressing the following primary issues:
- The type of caregiver(s) (CNA, Companion, etc.) best suited to assist the client
- The frequency and duration of caregiver visits appropriate for the client
- Assistance the client may require to maintain and/or increase social interactivity
- Consideration of additional services that could help the client feel more comfortable, happier, safer and ultimately more independent
Who Are the Caregivers Providing Custom Care?
- Certified Nursing Assistants (CNA)
- Personal Care Attendants (PCA)
- Certified Home Health Aides (HHA)
- Nursing Assistants (NA)
- Licensed Vocational Nurses (LVN)
The Care Manager assigns Astera Care caregivers who can administer a level of daily care appropriate for the client ranging from as little as 4 hours per day to around-the-clock care. In order to ensure the highest quality of complete care, we have implemented the following caregiver standards:
- All caregivers are bonded
- We conduct a complete criminal background check prior to hiring a caregiver
- We personally check references on all potential employees
- We have employment advancement incentives in place to promote exceptional service
- We treat our caregivers with the utmost respect and pay them accordingly
How is Custom Care Implemented?
Custom Care allows our clients to obtain at-home assistance at rates that are affordable without sacrificing the highest quality of care.
With the constant oversight of our Management team, the assistance of our Medical Advisory Board, and through affiliations with local health services companies, we enjoy and appreciate being able to provide our clients at-home care management services that will make their lives easier, more enjoyable and, above all, allow them to remain as independent as possible.
In-Home Assessment
Our first step is to introduce a Care Manager to the patient and family, and interview all appropriate parties. An Astera Care Care Manager will spend several hours in the patient’s home evaluating the patient’s physical, mental and psychosocial status, along with the patient’s complete medical history.
- Home Health Aide (HHA)
- Certified Nursing Assistant (CNA)
- Nursing Assistant (NA)
- Homemaker / Companion / Escort
Upon completion of this initial assessment, the Care Manager will provide a detailed Care Plan, recommending the most appropriate course of action for the patient, taking into careful consideration the patient’s wishes, the family’s concerns and the patient’s financial situation. Fees are approximate depending on level of care provided and number of hours scheduled.
- Astera Care’s Caregiver bill rates include nurse’s pay, general liability, malpractice, worker’s compensation, unemployment insurance and all local, State and Federal payroll taxes. Proof of insurance is available upon request.
- The following Holidays are billed at time and a half or double time: New Years Eve, New Years Day, Martin Luther King Day, President’s Day, Easter Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Columbus Day, Thanksgiving, Christmas Eve and Christmas Day.
- Each visit by a Caregiver is charged at a minimum of 4 hours.
Additional Services
Our a-la-carte.
We deliver Custom Care for each of our clients. Custom Care means something different to every client. For every case we complete an In-home Assessment, develop a Care Plan, offer around-the-clock crisis management, and interface with the client’s physician. However, Custom Care also means providing additional services, if and when requested. Just tell us what you want and we’ll provide services under your Plan.
Massage Therapy.
Whether our clients seek relaxation, muscle elongation, or just a soothing rub, we can schedule at-home visits by a professional masseuse. One-time or regularly scheduled visits are available. (Cost TBD per job)
Grocery Shopping Pick-Up & Delivery.
On a continual contract basis only, we offer our clients grocery pick-up & delivery. Our clients may request bi-weekly, weekly, bi-monthly or monthly grocery pick-up & delivery schedules. (Cost TBD per job)
Home Repair & Maintenance.
Through an affiliation with a local contractor, we offer our clients home repair & maintenance services. If our clients are in need of a safety rail, or simply ask that a showerhead be replaced, we will schedule a home repair visit. Continued home maintenance services are also available. (Cost TBD per job)
Transportation.
If a client is in need of transportation, we offer car transportation services, which include home pick-up & drop-off and caregiver attendance at desired destination.
Assistance with Calling or Writing Family Members.
If our clients would like assistance with staying in touch with family members, we offer regularly scheduled reminders and/or assistance with phone calls, postcards, letters, e-mails, etc. (Cost TBD per job)
Your Rights as our Client
Astera Care will protect and promote the exercise of your rights. Your rights as our client are listed below. If you have any questions about your rights or believe you are entitled to rights not listed on this document, please talk to your care manager or to an Astera Care representative, before or during the assistance provided to you.
Exercise of rights and respect for property and person.
- You have the right to have your property treated with respect.
- You have the right to voice grievances regarding Astera Care’s care that is (or fails to be) furnished, and you shall not be subjected to discrimination or reprisal for doing so.
Right to be informed of your care and right to participate in the planning of your care.
- You have the right to be informed of your care in advance of the care to be furnished, and of any changes in the care to be furnished.
- Astera Care shall advise you in advance of the disciplines that will furnish care, and the frequency of visits proposed to be furnished.
- Astera Care shall advise you in advance of any change in the plan of treatment or plan of care, or plan for personal care services, before the change is made.
- You have the right to participate in the planning of your care.
Confidentiality of Medical Records.
- You have the right to confidentiality of your clinical records maintained by Astera Care.
